To put your perspective in perspective, you are basing your conservativism argument on ONE small trial questioning QOL benefit of ablation - yet clearly demonstrating freedom from arrhythmia benefit - while we have multiple trials showing the opposite: with sham (see figure), AND with a moh early intervention (AFNET, CASTLE). It's good to always question, as long as you are honest about the answers.

@jeffrey_vinocur 2/ As we ((nature.com/articles/s4159…) and others have shown, the prevalence of type 1 Brugada pattern is highly underestimated, and accordingly the risk associated with isolated type 1 is likely overestimated.

@jeffrey_vinocur 1/ The key and fundamental issue is the overly permissive definition of "spontaneous type 1 pattern" as BrS. In the 2013 consensus statement (Priori), asymptomatic patients with spontaneous type 1 pattern required additional supportive features:

@drjohnm @DrRoderickTung We don’t ablate PVs “because we can”, but because in 1988 Michel Haissaguerre showed us that PVs are a fundamental source of AF. This ingenious work was reinforced by countless studies since. In contrast, what you are arguing is at best an unproven hypothesis. Just for context.

@drjohnm @JAMACardio @lusty63 Cardiac Output (CO) = Heart Rate (HR) x Stroke Volume (SV)

In #HFpEF , increasing EDV, and SV in turn, is limited by impaired relaxation. Thus HR is the primary mechanism for increasing CO in #HFpEF

#Pacing is aligned with increasing CO via HR. #betablocker therapy is not 👇
