It is hard to find an indication to implant pacemakers in the RV apex.
LBB Area Pacing vs RV Pacing for AV Block and Preserved Left Ventricular Function:
The #LEAP-Block Trial

📍499 patients with #LVEF ≥50% randomized to LBBAP vs #RVP across 14 Chinese hospitals.

📍 Primary endpoint: 2.4% vs 12.5% favoring #LBBAP (P=0.003).

📍 HF events: 3.4% vs .

📍 LBBAP ⬇️ the 24-month primary endpoint risk (HR 0.37, 95% CI 0.19–0.73), with no significant differences in mortality or device complications.

PFA was developed for PVI.
Any thoughts on durability anyone?

Among patients with #AtrioventricularBlock, preserved left ventricular ejection fraction, and anticipated high ventricular pacing burden, left bundle-branch area pacing reduced the 24-month composite of all-cause death, heart failure events, or upgrade to biventricular pacing vs g.

Device-related complications were similar between groups, supporting clinical consideration of left bundle-branch area pacing for patients expected to require frequent ventricular pacing.

Optimizing the right site of service is the future of healthcare. Coming to an ASC near you.
🚨 NEW SERIES ALERT 🚨 “ASCs and EP” is here!

Guidelines support AFib ablation. Patients want it. So why can’t they get in? 🤔

@DrRoderickTung and Dr. Rohit Malhotra talk about how lab capacity, not evidence, is holding care back, and how #AmbulatorySurgeryCenters (ASCs) could the care they need sooner.

Credit to the EP team in Bern for this excellent remapping study after either RF or PFA. Mandated remapping is much better than remapping for symptoms jacc.org/doi/10.1016/j.…

A) PFA no better than RF for durable PVI
B) Recapitulates GAP study showing that AF still occurs in one-third of durably isolated PVs and 72% of those with reconnected veins DID NOT have AF at one year

This is the crux of our problem in EP: we don't understand the pathophysiology of the condition. PVs get isolated because we can. @TobiasReichlin

Replying to @ChoiEueKeun
Where the evidence stands:
• High pacing burden → CSP prevents PICM; RCTs now support it as a primary strategy over RV pacing
• HFrEF + LBBB → CSP-CRT with confirmed capture
• AF “pace & ablate” → synchrony comparable to BiV pacing

Very excited to see 8 years of work coming soon @HRS_O2Journal. The rationale for dopamine is to avoid alpha agonism at all cost in HFrEF.

The majority of VT is mappable with anticipation, strategic positioning, and pharmacologic manipulation.

Congrats @ZJameria upadhyay!

Great kickoff with “Unknown EGMs” pre-symposium @APHRSOfficial in Korea. Preserving the art of intracardiac interpretation through Socratic method is not only fun but critical for the advancements in mechanism- most importantly, making the right diagnosis for patients.

5+ years in Phoenix, AZ.🌵🌞

My greatest joy? Building exceptional teams & bringing big visions to life. We come to work to change the practice of medicine - and elevate each other every step of the way.

• 4 interventionalists in 1 cath lab
• 3 imagers over 1 TEE
• 5 EPs on a ligeons, 1 IC, 1 EP on a fly fishing trip in Montana

At its best, medicine—and life—is a team sport.

How do you begin mapping a suspected septal substrate in non-ischaemic VT?

In this RIO Global 2026 live case, Roderick Tung and the team begin their electroanatomical assessment, taking an initial look at the right ventricle before moving on to map the left ventricle.

The team , using pace mapping to explore the region and compare findings with the patient’s morphology, while also considering a spontaneous PVC observed during the case.

Chair Julian Chun is joined by panellists Devi Nair, Ammar Killu, Andrea Sarkozy, Jason Andrade and Wendy Tzou for the discussion.

We are excited to welcome Dr. Deepa Mandale as our newest faculty member!

After completing her CV training @MayoClinic, she recently completed her level III Imaging and Advanced Echocardiography fellowship at @HeartUCSF.

She will add fire power to our growing non-invasive nal structural programs. @Sidsinecho @psorajja
