🫀From Healthy Atria to Atrial Failure: Rethinking Atrial Cardiomyopathy (AtCM) from the #HFA_ESC clinical consensus statement:
📍AtCM extends beyond #AF: Electrical, mechanical & structural abnormalities may precede clinically apparent AF or #HeartFailure
📍Diagnosis: Electrical atriscore ≥1) PLUS ≥1 of: mechanical dysfunction, atrial enlargement or excessive atrial fibrosis
📍Proposed diagnostic thresholds:
- P-wave ≥120 ms (standard ECG) or ≥150 ms (amplified ECG)
- Advanced interatrial block: P-wave ≥120 ms + biphasic morphology in ≥2 inferior leads
- LA reservoir strain <23%
- LAVi >40 mL/m² (>48 mL/m² in women >65 years)
- Atrial fibrosis: thresholds pending validation
📍New P-wave scoring system (0–4): Ranges from normal P-wave (0), prolonged P-wave (1), advanced interatrial block without (2) or with paroxysmal arrhythmia (3), to persistent atrial arrhythmia (4).
📍Atrial failure: End-stage AtCM, characterized by persistent AF or HF with symptoms attributable to atrial disease.
📍Early recognition matters! Risk factor & comorbidity management may help prevent or delay progression to atrial failure.

Certainly there is no evidence based outpt protocol. Every pt requires a personalized tx &amp; surveillance roadmap

Our proposed strategy advocates weaning off steroids quickly, PJP ppx, image &amp; screen for arrhythmias in selected pts, but to not overreact to any mild troponin ⬆️

While much attention has been focused on modifying the CD80/86 and JAK-STAT pathways in treating ICI myocarditis, we review immunomodulatory agents described in the literature.

We hope can this provide guidance as many may have limited access to "mainstream" tx like aba/rux

Dr. Richard Wright and I discuss the first peer-reviewed, real-world comparative effectiveness study of TTR stabilizers using U.S. claims data.

#Acoramidis was associated with a 34% lower risk of clinical r risk of diuretic intensificationcompared with #tafamidis.

In HFpEF, the randomized clinical trial evidence supporting the nonsteroidal mineralocorticoid receptor antagonist (MRA) finerenoneis more robust than that supporting steroidal MRAs, such as eplerenone.

Steve Greene and I examine the differences between steroidal and contrasting findings from randomized clinical trials, and the 2026 Expert Consensus Decision Pathway (ECDP) guidance on optimizing treatment with SGLT2 inhibitors and finerenone in patients with HFpEF.

The key takeaway? A shared mechanism of action does not necessarily translate into equivalent clinical outcomes. When making treatment decisions, it is the evidence for the individual agent, not simply the drug class, that matters.
