TAVIとかM-TEERとかのSHD系のインタべに循環器なりたてから興味あるのは結構。そのカテに入りたいとかって言う前にまず病棟の弁膜症患者の心不全管理とか、M-TEERが必要になる人の心不全の薬物療法が充分かどうか検討できるとか、エコーで重症度評価するとか、やるべきことはたくさんあると思うの。

Delighted to share our newly published Clinical Consensus Statement in the @ehj_ed European Heart Journal:

“Implementation of cardiac magnetic resonance imaging in everyday cardiology practice”

Our aim was simple but important: to make cardiac magnetic resonance (CMR) more ly useful to the general cardiologist and the non-CMR expert.

Rather than focusing only on how CMR images are acquired, we wanted to address the practical questions that arise in everyday cardiology:

🔹 What can CMR detect and assess?
🔹 How can CMR contribute to patient risk stratification?
🔹 What clinical question and information should be included in a CMR referral?
🔹 And, importantly, what should the general cardiologist look for in the CMR report?

We cover the clinical applications of different CMR sequences and the role of CMR across ischaemic heart disease, cardiomyopathies, valvular and aortic disease, adult congenital heart disease, pericardial disease and cardiac masses, as well as the increasingly important practical considerations around CMR in patients with cardiac implantable electronic devices.

The document also brings these applications into the context of current ESC guidelines and recommendations, with the ultimate goal of helping clinicians translate CMR findings into everyday clinical decision-making.

I hope this document will serve as a practical resource for cardiologists and help bridge the gap between advanced cardiovascular imaging and everyday clinical practice.

医局を辞めたい医師が本当に辞めたいのは、医局ではなく“人生を自分で決められない状態”かもしれない。

次の異動先が読めない。
子どもの教育設計が立てにくい。
外勤込みでようやく年収が成り立つ。
上司との相性で数年が決まる。
でも辞めたら専門性や人脈を失うのではと怖い。
だから悩む。
医局が悪い、辞めれば解決、という単純な話ではない。
でも、人生設計を全部医局に預けるのもかなり危うい。
医局を辞めたいと思った時、最初に考えるべきは退局届ではなく、
“自分は何から逃げたいのか”だと思う。

実際自分自身に当てはめて考えてみると興味深い。
異動先→ある程度家族を考慮して無理な異動はない。
年収→複業である程度成り立つ。
上司→結構恵まれている。
専門性→これが結構維持が難しくなる。専門医の維持は可能。でも本当にやりたい専門性のある仕事は難しい。

結果医局に残る方が得策だと考えた。
