Since concussions are typically diagnosed through self-reported symptoms, subtle symptoms can often be missed. So, doctors at NewYork-Presbyterian and Columbia Orthopedics have designed an AI- powered headset that can detect eye and neck movements that might otherwise be missed.

Partial knee Replacement has taken its place as a real, durable option for uni compartmental OA.
Being a non-surgical sports doc is more than concussions and ankle sprains.
It is knowing the evidence for surgical indications, best practices, and best surgeons.
And advising patients as such, so that when non-operative care is no longer their best option, the discussion can prepare them for coordinated surgical and post op care.
Tsukamoto et al, JBJS Am 2026. 2,234 consecutive UKAs, 85% revision-free at 15 years. Tech assistance, fixed bearing, and high volume: fewer revisions. Mobile bearing: more. Top reason: arthritis in the adjacent compartment. Single-center retrospective. pubmed.ncbi.nlm.nih.gov/42987190/

Which partial knee lasts? In 2,234 UKAs followed to 15 years, revision-free survival was 85%. Tech assistance, fixed-bearing design, and high volume were linked to fewer revisions; mobile bearing to more. How you do the case matters as much as which implant.
Choose the best surgeon for the job.
Don’t be afraid to ask how many your surgeon has done with and without robotic assistance.
What their outcomes are in terms of successful return to activity and failures/complications. And never forget to ask if the surgery is necessary at this time or if there are viable non operative options.
In the same vein don’t delude yourself into calling surgery a “last resort”. The risk of doing surgery needs to be balanced against the real expected outcomes without.

Training for life means finding the right balance between effort and recovery.
The 80/20 training rule recommends keeping approximately 80% of your workouts at a relatively easy intensity and 20% at a higher intensity. This approach can help athletes and active individuals maintain consistency without pushing their bodies too hard.
Dr. Thomas S. Bottiglieri, DO, explains how balancing workout intensity can support long-term fitness and help reduce the risk of athletic injuries.
Watch the video to learn how to train smarter, stay consistent, and keep moving toward your goals.

A positive early VOMS roughly doubles the odds of a long concussion recovery. Useful signal, but not the whole exam. On the sideline I still want the full symptom inventory, cranial nerves, neck, and a Romberg. VOMS is one piece of a multimodal screen.

Rahmaniharedasht et al, JNPT 2026. Systematic review and meta-analysis, 12 studies (N≈3,300; meta n=968). Positive early VOMS: OR ~2.4 for prolonged recovery. Isolated VOR less consistent after adjustment. Specialty journal, so read with care.

Consistency is key when it comes to long-term fitness and athletic performance. But training harder every day isn't necessarily the answer.
Dr. Thomas S. Bottiglieri, DO, explains the 80/20 training principle, where approximately 80% of workouts are performed at an easier intensity and 20% at a higher intensity. This approach can help athletes balance training demands while working toward their goals.
Whether you're running, weightlifting, or simply training for lifelong fitness, the goal is to stay active, avoid injuries, and keep moving.
Watch the video to learn how a balanced approach to training can support your long-term fitness journey.

Does the software programming in your brain affect your recovery after a musculoskeletal injury? Neuromuscular reintegration is absolutely essential in your rehabilitative program. Six weeks minimum to reprogram movement before hypertrophy occurs.

@mubashirali822on models of communication and patient instructions/follow ups for years.
The process is simple.
Put patients first everyday. Find inefficiencies in eliminate them so that you have the time to manage patient care.
Most docs have been reduced to data entry clerks. Focus on what is important.

100%. See Singapore system….
@Benefitsguynick like to see a government option for only catastrophic coverage , let me pay everything else out of pocket .
And sure we have med share but I think this would be a more realistic solution , at the same time could check and balance the private payers as well as they would need to compete

Perfect is the enemy of good. In sports and fitness, striving for perfection can sometimes work against your progress.
Whether you're a student athlete, training for a marathon, or working toward a personal fitness goal, it's important to recognize that every body responds differently to training. Success isn't about comparing yourself to others. It's about setting realistic goals, respecting your limits, and listening to what your body is telling you.
Dr. Thomas S. Bottiglieri, DO, shares why progress matters more than perfection and how meeting your body where it is can support a healthier approach to athletic performance.
Watch the video for a reminder that you don't have to be perfect to make meaningful progress.

Optum Health aka United is dropping doctors it calls not "appropriately aligned."
UnitedHealth employs roughly 9,000 physicians and contracts with about 80,000 more.
Fair enough. A buyer can choose its suppliers.
If your doctor sold to Optum, someone in Minneapolis now decides whether she's aligned.

They are mutually exclusive.

MRIs are a great example of a service that is to cheap to insure.
If you insure services that are too cheap to insure, your insurance becomes unaffordable.
That is the flaw of the ACA that no one wants to admit!

Personalized titration matters.
1/2 -- A throwing program is not a countdown. It is a workload prescription.
Return to throwing cannot be defined by time alone.
Our research team used biomechanical workload data from 238,611 throws by healthy collegiate pitchers to develop interval throwing frameworks across , 12 weeks, 5 months and 7 months.
The principle connecting them is simple: rebuild throwing capacity through a deliberate progression of distance, volume and intensity while limiting abrupt workload spikes.
But data does not clear an athlete.

Fascinating article in Journal of Personality and Social Psychology by Justin Kruger and David Dunning from 1999. Healthcare executives, lawyers, economists, and policy wonks must read it!
The cruel irony of incompetence is that the very knowledge required to recognize it…is incompetent lack.
Competence reveals complexity; and, incompetence conceals it. As it turns out, the less you know…the less you know what you don’t know.

PRP 💉 is one of the things I get asked about most.
My answer: how it's made and how it's given matter as much as the shot itself. A new trial in early knee arthritis found a standardized approach beat "however each doctor does it" on pain, function, and side effects.

Yang et al, AJSM 2026. RCT, 240 pts, KL I-II knee OA. Leukocyte-poor PRP + ultrasound guidance + structured rehab beat usual-practice PRP at 1 yr, ~half the adverse events, similar cost. No placebo arm: it shows protocol matters, not that PRP works.

Rest 😴in a dark room 🛌 isn't the treatment for a concussion 🤕 that lingers.
Exercise is medicine. 💊
A new trial in teens still symptomatic weeks out: a heart-rate-guided aerobic program, coached remotely by video, got them better faster than stretching. Start early, dose by heart rate. 🫀
