Some risk factors announce themselves.
Others have been hiding in the pitcher’s hand all along.
In a study of 189 professional baseball players, those who had undergone Tommy John surgery had significantly smaller hand spans. Two measurements stood out: thumb-to-pinky span and dex.
Why might that matter?
A smaller hand may have to work harder to create and maintain the same grip. Greater muscular effort can accelerate fatigue in the flexor-pronator muscles, the dynamic shield helping protect the elbow.
When that shield tires, more stress reaches the UCL.
Among the smaller-handed players under 21 studied, 57% had already undergone UCL reconstruction.
That number should not label an athlete or determine their future. Hand size is not destiny. It is information, and information gives us an opportunity to plan more intelligently.
You cannot change the hand.
You can change the plan.
Early screening. Individualized pitch design. Coaching built around the athlete in front of you, not the athlete a system assumes.
The risk may be written in the hands.
The response belongs to all of us.

1/2 -- THE NAME ON THE DOOR MAY BE MINE. BUT THE STANDARD BELONGS TO ALL OF US.
We recently shared a simple truth: no comeback has one author.
Today, I want you to meet some of the people helping our patients write theirs.
Great sports medicine is never one appointment, one n. It is a connected journey built through expertise, communication, education, precision, and trust.
Frank Alexander, MS, ATC, is the bridge between treatment and recovery, evaluating athletes, answering the difficult questions, guiding postoperative care, and serving as a dedicated Tommy John Coach.
Billy Wagner, MS, PA-C, helps connect the clinic, operating room, and recovery process. His clinical precision and experience as a former collegiate athlete allow him to understand both the medical journey and the mindset required to return.
Yael Simpser, DNP, FNP-BC, brings individualized, athlete-centered care to every stage, helping diagnose and manage injuries, navigate complex cases, and support patients throughout rehabilitation.
Different roles. Different strengths. One shared standard...

Getting better begins with the willingness to look honestly at your own performance.
The conversation you could have handled better. The preparation you rushed. The feedback that was uncomfortable to hear.
Each offers an opportunity, if you’re willing to examine it.
In adership, experience becomes more valuable when we reflect on it, seek feedback, and make specific changes to how we work.

1/2 -- A throwing program is not a countdown. It is a workload prescription.
Return to throwing cannot be defined by time alone.
Our research team used biomechanical workload data from 238,611 throws by healthy collegiate pitchers to develop interval throwing frameworks across , 12 weeks, 5 months and 7 months.
The principle connecting them is simple: rebuild throwing capacity through a deliberate progression of distance, volume and intensity while limiting abrupt workload spikes.
But data does not clear an athlete.

1/2 -- The finish line marks the end of the race. It does not mark the end of the medical plan. Congratulations to the 2026 NYC marathon runners...
After 26.2 miles, soreness, stiffness and fatigue can be expected. The important question is how those symptoms behave next.
Are g, or becoming more focused, more intense or more limiting?
Post-marathon recovery begins by rebuilding the basics:
💧 Rehydrate according to your individual needs
🥗 Refuel with balanced nutrition
😴 Prioritize quality sleep
🚶 Introduce gentle, low-impact movement
Pay attention to pain or swelling that is not improving, a new limp, altered movement, instability or loss of motion. Those changes deserve a proper evaluation.
Chest pain, severe breathing difficulty, fainting or confusion require urgent medical attention.
There is no prize for racing through recovery. Respect the work your body has performed, allow it time to respond, and return to running gradually.
Recovery is part of the achievement.
Save this carousel and share it with a runner who earned their medal this weekend.
**Independent runner education. This post does not diagnose an injury. Follow individualized guidance from your healthcare team.

An injured athlete brings more than an injury into the room. They bring uncertainty about their season, their future, and a sport that helps define who they are.
“Sit down next to them and talk to them eye-to-eye.”
— Dr. Christopher Ahmad
That simple act matters. Take a seat. Explain clearly. Give difficult news with honesty, and leave room for questions.
The athlete deserves to understand what comes next, and to know they have someone beside them through it.
Trust is built in these moments.
Inspired by Dr. Ahmad’s interview with Columbia Magazine, “MVP: Most Valuable Physician.”

A radar gun shows velocity. It cannot show what the body had to overcome to produce it.
Cold tightens muscles and reduces grip feel. Heat drains endurance. Humidity changes the baseball. Altitude changes pitch behavior.
Then the smallest environment takes over:
Slicker ball → arm activation → more elbow torque.
The pitch may look the same. The physiological cost may be completely different.
“The UCL does not know where it is. It only knows what it is being asked to tolerate.”
The same workload is not always the same workload.
Geography is destiny—until you plan for it.
