Effects of Acoramidis on Kidney Function in Transthyretin Amyloid Cardiomyopathy

Acoramidis initiation resulted in an acute dip in eGFR and reductions in both the chronic eGFR slope and urinary albumin-to-creatinine ratio versus placebo without adverse kidney-related events. idney function may be mediated through direct kidney-protective hemodynamic effects.

🫀 Cardiac PET Quiz #01

86F referred for dyspnea.
Rb-82 PET, regadenoson stress.

• Perfusion: normal
• No TID, no EF drop
• Heavy multivessel CAC on the CT
• Global MFR 1.12 😬

Balanced 3VD? Not so fast.
The spleen switches off at stress.

What would you check next?

A) Go b
B) Hemodynamics during stress
C) Caffeine history
D) Treat as CMD

Beyond coronary stenosis: How can we better predict cardiovascular risk?

Excellent Grand Rounds today at @BrighamWomens with Dr. Bernard Gersh. @BernardGersh

Key insights on plaque burden, vulnerable plaques, the PREVENT trial, and advanced CCTA imaging.

The future lies in ysiology, and inflammation.

A few takeaways 🧵👇

🫀 Can clinically important cardiac sarcoidosis occur without LGE on CMR?

Our new short report in European Heart Journal – Imaging Methods and Practice addresses this question.

The added value of FDG-PET when LGE is absent has been questioned.

🇨🇦 In a single-centre substudy of S cohort, we examined 46 treatment-naïve patients with clinically manifest cardiac sarcoidosis and core-lab-defined abnormal cardiac AND extracardiac FDG uptake.

🔎 What did we find?
4/46 patients (8.7%) had no LGE
All four presented with advanced AV conduction disease

"No LGE does not necessarily mean no clinically important cardiac sarcoidosis."

⚠️ This small, selected cohort does not establish poor long-term prognosis or justify PET for everyone without LGE. It supports considering FDG-PET when clinical suspicion remains high despite absent LGE.

Grateful to all our coauthors for this work, and looking forward to the CHASM-CS RCT results on treatment strategies!

Transforming systemic amyloid (ATTR and AL) into treatable diseases (not curable yet). Less than 9 years ago there were no proven treatments for ATTR, and limited options for AL, now we are debating combo vs mono, different lines of therapy, earlier identification, when should we treat vs observe, running prevention trials and highly specific PET tracer trials.

Still lots of work to do. Depleters, imaging, biomarkers, rare amyloids, light chain stabilizers, new silencers, gene editing ......
With amyloid-depleting therapies on the horizon, imaging may play a larger role in assessing treatment. The panel discussed a key research need: linking amyloid burden with disease trajectory and biomarkers to identify which patients are progressing. #AmyloidosisForum

CARDIO-TTRansform narrowly missed its main goal overall, but the results tell a more nuanced story. What did we learn about eplontersen, stabilizer use, and combination therapy in ATTR-CM?

Our first session is underway with @JLGrodin discussing advances in ATTR amyloidosis and the challenges that remain. The shift from biopsy-based to nonbiopsy diagnosis is creating opportunities to identify patients earlier.

Our first moderated discussion tackles ATTR trial design: Do composite endpoints capture residual burden and when treatment benefit emerges? Are hard outcomes improving? How does enrichment affect generalizability? #AmyloidosisForum

Composite endpoints should reflect patients’ experiences. In systemic ATTR, quality of life is shaped by more than heart failure or heart function. Measures that worked in earlier trials may need to evolve with today’s treatment landscape. #AmyloidosisForum

After a decade of imaging patients in amyloidosis trials, what would it take for the next five years to change care? The panel called for turning the imaging data we already have into measures that can guide treatment and show what matters to patients. #AmyloidosisForum

Could treating ATTRwt before significant symptoms prevent organ damage? How do we identify who would benefit? The panel emphasized the need for more data and prospective trials to answer these questions for patients with minimal or no symptoms. #AmyloidosisForum

When is a trial score change meaningful to patients? Are within-patient changes meaningful, the anchors valid, and composite domains moving together? These questions are vital when a patient-reported measure is the primary endpoint. #AmyloidosisForum
