@docwarsi Misperception of risk, lack of urgency, outdated algorithms, inertia, and misaligned incentive are major drivers
Many system based solutions exist, yet are not implemented at scale
Changes in policies could potentially incentivize improvements

Incentivize practices for getting HFrEF patients on all four pillars, and set a target that rewards progress over perfection
That's how a policy can turn the HF guideline into care
If only @CMSGov weren't twenty years behind in HF measures

Heading to AHA Scientific Sessions 2026 in Chicago?
I’d love to connect with colleagues interested in what’s next for finerenone in heart failure on Friday evening.
I’m extending an open invitation to investigators, clinicians, and research teams to join us at the University n how the MOONRAKER program is moving forward—and explore opportunities to contribute to this work.
Building on FINEARTS-HF, three complementary phase 3 trials are addressing questions that matter in everyday clinical practice:
• REDEFINE-HF: Evaluating whether finerenone reduces cardiovascular death and first and recurrent HF events in patients hospitalized with acute heart failure and LVEF ≥40%. This will extend the evidence into the vulnerable period surrounding an HF hospitalization.
• CONFIRMATION-HF: Testing an early treatment strategy combining finerenone with an SGLT2 inhibitor across the EF spectrum, assessing whether this approach improves clinical outcomes and patient-reported symptoms compared with usual care following an HF hospitalization.
• FINALITY-HF: Evaluating finerenone in patients with HFrEF who are intolerant of or ineligible for steroidal MRAs, generating evidence on cardiovascular death, HF events, symptoms, and safety in a population with an important unmet treatment need.
Together, these studies will clarify where finerenone fits across HF populations and care settings, including its role in early combination therapy and in patients with limited treatment options.
I’m proud of our Saint Luke's Mid America Heart Institute team’s collaboration with Marc Bonaca, CPC Clinical Research, and investigators worldwide to advance this program.
Whether you’re already involved in MOONRAKER, interested in research collaboration, or simply want to learn more, please join us for an evening of conversation and connection.

Multiple roommates. Ramen noodles. Near public transportation. Respect to the people who do whatever it takes.
I worked as a bartender. Bar-back. $2.15 an hr plus whatever tips got shared. Worked at a retail store. Never had benefits at any job.
Had 5 roommates in a 3 br apt. Ate lousy food out of cans and ate a ton of peanuts and shit food at happy hour. Had credit cards ripped up. Etc.
But I learned a ton. Taught me what was right and wrong about running a business by watching my bosses and their bosses
Yeah I know. It was much easier then, when inflation was 15 pct and national unemployment was 10 %. I know I was fortunate when I finally got fired from the store, and had to start my own business with zero cash. Bill collectors everywhere.
Even luckier when I found my first customer so I could pay my bills
But I recognize it’s so much harder today.
Which is why I respect the people who are willing to do what it takes to get started

Everyone has to start somewhere.
Take a risk. Put yourself out there.
And it’s great to give people a chance.

Before Hollywood, I was living in a van in Hawaii and waiting tables at Bubba Gump Shrimp Company. One day, the incredible Rae Dawn Chong walked into the restaurant, and naturally, I went up to her table and said, “Hey, you’re a movie star! What are you doing here?”
She asked if , “Fuck yeah, put me in your movie!”
She asked for my phone number. The only problem was I didn’t have one. So I gave her my buddy Mike’s number instead. A couple days later, Mike casually mentions, “Oh, by the way, someone called you.” ARE YOU KIDDING ME, MIKE?!
I called her back, and sure enough, she was serious. I drove over to pick up the script, went back to Mike’s house, and read all 120 pages three times in a row. First script I’d ever read. My character's name was Devon, and I remember thinking, “Sweet! I could totally play a Devon!”
I showed up to the audition with every line and inflection mapped out in pencil. Rae told me I could act and offered me the part. I asked if we were shooting in Hawaii. Nope. Los Angeles.
I had about $60 to my name, a bag of clothes, and some rollerblades.
Then Rae told me they'd fly me out. And I remember thinking right then and there, my life just changed forever. I'm going to be an actor.
That little movie was Cursed Part 3, a low-budget horror comedy that opened the door to Hollywood for me.
I'll forever be grateful to Rae for taking a chance on a kid working at a shrimp restaurant. Everybody starts somewhere, and this is where it all began for me.

Can septal myectomy for obstructive HCM be done safely and effectively outside a high-volume center?
97% of high-volume centers are in large metropolitan areas. At LVHN, one surgeon performed 146 septal myectomies from 2006 to 2023 for patients iles away. Outcomes compared favorably with national series.
The authors’ takeaway: at lower-volume centers, concentrating septal myectomy in a single operator may help build and keep expertise, and bring HCM surgery closer to home.
