🔥 Beyond placebo!
Does AF ablation really improve quality of life?
Our meta-analysis of sham-controlled RCTs shows that with AF ablation:
✅ +13.5 AFEQT points
✅ 51% lower AF recurrence
✅ Benefit beyond active-treatment controls
....and this is with SHORT follow-up and an rol!

Proud of @MekhaelMario for leading this DECAAF-II analysis! Our findings echo @peterkistler3’s work on AF burden, with an important distinction: LA volume also independently predicted arrhythmia recurrence after PVI.
LA enlargement offers a window into atrial remodeling. In our cohort, larger atria had more fibrosis, suggesting more advanced structural disease—a substrate that may sustain #AFib despite #PVI
Understanding the underlying atrial disease is central to understanding ablation outcomes. Actual current priority focus of @TulaneHeart and @TulaneTriad @Phiso_de @chris_sohns @PrashSanders @gautamsand @S_NarayanMD @TinaBaykaner @Siddmann @drjohnm

@peterkistler3 @DrJeremyWilliam Very interesting study! We found quite similar findings in our DECAAF-II subanalysis, where LA volume and AF burden were correlated with a similar R² of 0.12. However, we also found a significant difference in AF recurrence rates between patients with low versus high LA volumes.

While slowly making my way home from #APHRS2026 in Busan South Korea I’ve been reflecting on the amazing week we’ve shared together. Our EP community is filled with so many amazing people pushing frontiers and making incredible discoveries that push our field forward. It often appening and it’s hard to keep up. Which is why it’s so important that meetings like APHRS bring us all together, to share in this information and showcase the incredible work that’s being done around the world. I’m honoured to take part in this incredible meeting and so happy that I was able to share this experience with old and new friends. This first trip to South Korea has been incredible and I look forward to many more.

Proud to be part of this work led by @PrashSanders . Pooling all 3 sham-controlled RCTs (SHAM-PVI, PFA-SHAM, PVI-SHAM-AF): AF ablation beats sham. QoL +13.5 AFEQT points, 94% probability of a clinically meaningful gain. #AFib recurrence cut in half (RR 0.49), 99% probability of benefit.@DrJasonAndrade @Mel_Gunawardene @peterkistler3 @lamajebbawi

@PrashSanders @Dominik_Linz @DJ_Lakkireddy @atulverma_md @JonPicciniSr @True_EP @AndreaRussoEP @purerfellner @joselmerino @drrakeshg1 @Ed_Gerst Great news! The 17 years of ablation for symptomatic AF despite AAD that I was 100% sure worked at a minimum to improve QOL and reduce CHF and possibly SSE turns out not to be a wasted effort. Probably could have figured this out with patient testimonials. Love it. 😍

Proud to have been part of this important collaboration.
As mentioned back in August when the results were presented at ESC - it’s always important to consider the evidence base and nuance within each trial.
Top line results don’t tell the whole story (see PIF-PAF) nor does one study mean we forget everything that came before it.

This:
CSP followed by atrioventricular node ablation is an effective rate-control strategy in atrial fibrillation, offering ventricular synchrony and haemodynamic benefit similar to that from conventional biventricular pacing.
