I’ve always enjoyed listening to the @GIE_Journal Podcast, so it was a pleasure to join @DouglasAdlerMD for a discussion about a question that comes up more and more in advanced endoscopy: Should every advanced endoscopist be doing everything?

We trained in very different times, and it was interesting to compare our experiences and reflect on how much the field has expanded. Advanced endoscopy now encompasses such a broad range of procedures that it raises important questions about training, developing expertise, maintaining quality and safety, and whether one person can or should truly do it all

A great conversation about where the field has been and where it may be heading.
Check out the latest podcast: GIE Editor-in-Chief Dr Douglas G. Adler chats with Dr Ramzi Mulki in "Can you do everything in endoscopy? Should you do everything in endoscopy?" giejournal.org/audio-do/can-y…

The next generation is here! The first batch of first year fellows is in the house to absorb all things GI and endoscopy. Hearing from seasoned experts, getting hands-on experience and making connections......it's all happening now at ASGE's IT&T.

The future is 🔥🔥!

I'm excited to share that I'll be joining the University of Alabama at Birmingham's Advanced Endoscopy group. It's a privilege to practice alongside such a collaborative and talented team, and I'm looking forward to the work ahead.

Replying to @JAMASurgery
@JAMASurgery Yes but in the modern world the average general surgeon would rather walk uphill barefoot a mile in the winter to avoid a common bile duct exploration.

In our multicenter prospective study we show that sending FNB samples for histopathology had similar diagnostic yield compared to cytology. Ability to offer EUS-FNB may not need to be strictly tied to the presence of a cytopathologist at the institution. @bcm_gihep

Can we safely offer colorectal ESD to elderly patients in Western settings?

In our study of 710 patients undergoing colorectal ESD @StLukesHealthTX and @bcm_gihep , we found:

🔹 Comparable en-bloc resection rates across age groups: <65 yrs: 87.4% | 65–75 yrs: 80.9% | ≥75 yrs: 80.st-procedure adverse event rates:
<65 yrs: 6.7% | 65–75 yrs: 8.5% | ≥75 yrs: 6.3%

🔹 Slightly longer hospital stay in ≥75 yrs (1.1 days), but still shorter than Eastern cohorts.

Despite higher Charlson scores and more antithrombotic use, outcomes in the elderly remained robust.
🧠 This is the largest Western and US cohort to date assessing colorectal ESD in the elderly and supports its broader use in aging populations when performed by expert endoscopists.

Congrats @FaresAyoubMD on leading this excellent study.

There are 3 methods to stop diverticular bleeding - indirect vs direct clipping, or banding.

While direct clipping is suggested to be superior, it’s hard!

If bleeding is in L colon,
I perform band ligation. Ask for it pre-installed on another scope to save time.
