Here’s how you can achieve this enhancement using a Rotablator:
1. Take an introducer needle and place the CP12 alongside a Rota wire.
2. Sharpen the tip of the CP12 against the rotating burr (1.75mm) of the Rotablator.

To shorten a guide catheter:
Trim 10–15 cm from the guide’s proximal end. Use a femoral sheath 1 Fr smaller than the guide. Cut a 2 cm sheath piece as adapter, flare both ends and slide adapter over the cut ends of the guide.

Anticipation and prevention are a key to avoiding complications, but when they happen, being aware of principles and being familiar with a management algorithm is key.  Burr/crown entrapment is rare, and burr/crown loss is even rarer. Regardless, all those performing atherectomy should be aware of this potential catastrophic complication and know how to deal with it! This is a must read for all those performing atherectomy.

Our microcatheter anchor technique (MAT) published. Working around retroflexed graft insertion sites can be difficult. MAT adds to our current balloon anchor toolbox (eg BASE and side BASE) and can be used for wiring, MC tracking in other difficult coronary anatomies.
