In 2014, I started recording simple teaching videos for medical students. Built a website to stream the content, put it behind a paywall, and slowly grew the library over years. By 2022, it was generating millions in revenue and I sold the company to McGraw Hill where it’s now publicly traded on the NYSE.

American politics has its flaws, but I have always found election nights inspiring. The candidates have to sit back and watch as the people have their say.

Sarah Longwell holds focus groups across the US. She said on the Ezra Klein podcast that more and more Americans are looking for a universal healthcare option. She said Medicare for All polls higher than ever, and even Trump voters want major change to US healthcare.
If Democrats win both houses of congress tonight then the White House in 2028, this could become reality. The first steps toward a sea change for US healthcare could be happening tonight.

Nobody knows the Indigo Girls any more or their 90s hit, Closer to Fine, but the chrous is a life lesson for the ages that comes back to me again and again as I get older: “…there’s more than one answer to these questions pointing me a crooked line; the less I seek my source for some definitive, the closer I am to fine.”

Oh, man. This hits hard. Next Generation was huge in the 90s. Incredible actor.
Patrick Stewart, who will be remembered most for his roles as Captain Jean-Luc Picard on “Star Trek: The Next Generation” and as Professor Charles Xavier in the “X-Men” films, has died at 86.

Per NYTimes, Gen Z job seekers say employers now require them to record answers to interview questions on their phones and submit the recordings for AI chatbot review. Are any medical residency programs trying something similar? Sounds miserable for applicants but time saving for employers.

This app switched me from Following to For You and the first five posts had me so angry I got hypertensive crisis with TIA symptoms.
For You should be 2 points in the next CHADS score revision. Go on Apixaban if you’re going to scroll it regularly.

Practice questions on the Boards and Beyond website have been answered 9.5M times by students the past year. This is absolutely the best way to follow-up watching the videos.

Trump must hate Obama so much because Obama stays out of the limelight he doesn’t crave attention like Trump but when he does enter the fray he skewers Trump and goes viral in the way Trump dreams of. Obama is everything in terms of branding Trump can never be.
Obama did a 5-minute comedy masterclass on the Cheesecake Factory MAGA meltdown at the Ossoff/Bottoms rally tonight. Absolutely incredible.

The first Diagnostic and Statistical Manual of Mental Disorders, DSM-1, was published in 1952. It had about 100 disorders compared to over 300 in the current DSM-5. There were no checklists for diagnoses just a vague description of which symptoms went with a given disorder. It wasn’t very useful but paved the way for DSM-3 in 1980 which is when the modern system of checklists for diagnosis was developed.

The key thing to know is the DSM has always categorized disease by symptoms. Few other fields in medicine do this. Symptoms of chest pain disorders often overlap but we distinguish them from one another with diagnostic tests like EKG, CT scan, labs. Same for disorders of dyspnea or abdominal pain. Not so for psychiatric disorders per the DSM.

I’m watching The Dropout on Hulu about Theranos and Elizabeth Holmes and maybe this is all hindsight but I can’t believe people weren’t more skeptical of the tech. She had zero scientific or medical background. Sad commentary on human gullibility.

This video by @jbcarmody should be mandatory viewing for anyone in medical school or residency. Fantastic explanation of the history of US residency training. Why is it so long? Why so many hours? Everyone going through it should understand how this came to be. youtu.be/exzYrrCjf88?is…

In general, I hate the bureaucracy associated with health insurance. But every time I take my car to get repaired, I also worry about what pure fee-for-service medicine might look like.
"Mr. Jones, we have several packages for your hip repair. The bronze is fine, nothing wrong with it, but if you want to feel truly light on your feet you should get the silver which is on special this week. And I highly recommend one of our protection plans in case your hip breaks again down the road. They're 10% off if you sign up for our credit card today."

Took my car to a local repair shop for squeaky brakes and, of course, they told me it needed pads, tires, coolant, etc. all for a lot of money. After I picked it up, I put the bill into ChatGPT. It found an errant double charge ($200) and a strange tire “maintenance fee” ($160). Called the shop and got all this stuff refunded.
