Excited to share our work on aspirin for primary prevention in ppl w/ elevated Lp(a) @JAHA_AHA @AHAScience

In a propensity matched cohort in MESA, aspirin was a/w a 46% reduction in CHD risk in ppl with Lp(a) >50 mg/dL.

🫀 Can coronary atherosclerosis actually regress?

Our new randomized FLOWPROMOTE trial analysis, published TODAY in JAMA Cardiology, shows remarkable coronary plaque regression with intensive LDL-C lowering!

Atherosclerosis is not necessarily a one-way process.

In our new @masala_study analysis @JACCJournals, Lp(a) levels vary among disaggregated South Asian groups, but the association between Lp(a) levels and outcomes does not.

We tested over 30 new ways of calculating a CAC score to incorporate the inverse association between plaque density and risk and develop CAC-VD. CAC-VD demonstrated improved performance over the Agatston score in MESA and an external validation cohort.
Can we improve on the Agatston score? 🫀 The new CAC Volume & Density (CAC-VD) score is simple to calculate, accounts for the inverse association of calcium density with CHD risk, & improves risk prediction vs the Agatston score @hsbhatia @UCSDCards

Session 2 of Day 2 of the @FamilyHeartFdn Global Summit kicks off with a great talk by @hsbhatia from @UCSDCardiology on Lp(a), post Horizon:
✅ Still a powerful risk enhancer for CVD
✅ Risk favor management reduces risk
✅ Screening still critical

Is low Lp(a) associated with risk of diabetes? This new study @AJPCardio suggests it does not, based on a SNP associated with very low Lp(a) levels.

It remains unchanged that Lp(a) is a well established risk factor for CVD and knowledge of Lp(a) levels helps to better understand personalized risk and can drive more aggressive risk factor management.

CAC can be effective for risk stratifying individuals with elevated Lp(a). In this study, we ask when to start CAC scoring and when repeat scoring should be considered.

I laugh because you obviously read the paper which talks primarily about the association of Lp(a) with atherosclerosis in multiple vascular beds, the potential role for CAC, and the uncertain role of CCTA and carotid US. Thanks for taking it so seriously :)
Glad to entertain you, I guess!

However, seasoned doctors have all seen the harms of low value testing cascades and take these things more somberly

Great essay I just linked in my feed talking exactly about these harms
