🚨⭐ #ESMO26 | GU ONCOLOGY — DAY 2 HIGHLIGHTS

A major day for bladder and prostate cancer: practice-relevant phase III data, longer survival follow-up and new molecular clues.

🟠 VOLGA | LBA2 — A pivotal MIBC result

In cisplatin-ineligible/refusing MIBC, perioperative t enfortumab vedotin vs radical cystectomy ± approved adjuvant therapy:

✅ 2-year EFS: 64% vs 45% | HR 0.51
✅ 2-year OS: 78% vs 69% | HR 0.58

A meaningful survival signal, but not a head-to-head comparison with EV + pembrolizumab.

🔵 DASL-HiCaP | LBA4 — Intensification with radiotherapy

Darolutamide + ADT + RT improved 4-year metastasis-free survival to 95% vs 88% (HR 0.42) in very-high-risk prostate cancer.

OS remains immature; this use is investigational.

🔵 PEACE-1 | 2215O — Durability matters

After 7.7 years' follow-up in de novo mCSPC, ADT + docetaxel + abiraterone delivered median OS of 6.2 vs 4.4 years (HR 0.68).

Eight-year OS: 40.3% vs 27.1%.

🔵 PROTEUS substudy | LBA53

Perioperative apalutamide + ADT around prostatectomy vs surgery alone in high-risk localised disease:

✅ EFS HR 0.38 (n=408).

Promising but setting-specific evidence; longer follow-up matters.

🧬 STAMPEDE / Decipher | 2045O

Genomic risk may help identify patients deriving greater benefit from abiraterone for distant-metastasis prevention.

Retrospective biomarker evidence—not yet a mandate to de-escalate.

🟠 NIAGARA molecular analysis | 4741RO

Exploratory EFS signals across several molecular subtypes, including basal-squamous disease.

No validated biomarker yet to withhold perioperative durvalumab.

🟠 EV-103 | 4740RO

Long-term EV + pembrolizumab follow-up in advanced urothelial cancer:

✅ Estimated 5-year OS: 33% in pooled cohorts.

Durability and late toxicity both matter.

🟠 GDFATHER-NEO | 4757RO

Visugromab + nivolumab:
✅ pCR 33.3% vs 7.7% with nivolumab + placebo.

Exciting but exploratory (n=28 evaluable).

⭐ MY CLINICAL PEARL

Not every form of intensification is equal.

What matters is proven survival or metastasis prevention, alongside toxicity, patient selection and quality of life.

Precision AND durable benefit—not just higher response rates.

🚨⭐ #ESMO26 | GU ONCOLOGY — DAY 2 HIGHLIGHTS
@urologysummit
A major day for both #BladderCancer and #ProstateCancer — with one message coming through clearly:

intensification only matters when it delivers durable, clinically meaningful benefit.

🟠 VOLGA | LBA2 — MIBC
Perioperative durvalumab + neoadjuvant EV in cisplatin-ineligible/refusing MIBC:

✅ 2-year EFS: 64% vs 45% | HR 0.51
✅ 2-year OS: 78% vs 69% | HR 0.58

Important survival signal — but not a head-to-head comparison with EV + pembrolizumab.

🔵 DASL-HiCaP | LBA4
Darolutamide + ADT + RT in very-high-risk prostate cancer:

✅ 4-year MFS: 95% vs 88% | HR 0.42

OS remains immature; still investigational.

🔵 PEACE-1 | 2215O
After 7.7 years of follow-up in de novo mCSPC:

✅ Median OS: 6.2 vs 4.4 years
✅ 8-year OS: 40.3% vs 27.1%

This is what durable treatment intensification looks like.

🔵 PROTEUS substudy | LBA53
Perioperative apalutamide + ADT around prostatectomy:

✅ EFS HR 0.38

Promising — but longer follow-up and setting-specific interpretation are essential.

🧬 STAMPEDE / Decipher | 2045O
Genomic risk may help identify who derives greater benefit from abiraterone for distant-metastasis prevention.

Interesting biomarker signal — not yet a basis for de-escalation.

🟠 NIAGARA molecular analysis | 4741RO
Exploratory EFS signals across molecular subtypes, including basal-squamous disease.

Still no validated biomarker to withhold perioperative durvalumab.

🟠 EV-103 | 4740RO
Long-term EV + pembrolizumab follow-up:

✅ Estimated 5-year OS: 33%

Durability matters — and so does late toxicity.

🟠 GDFATHER-NEO | 4757RO
Visugromab + nivolumab:

✅ pCR: 33.3% vs 7.7%

Intriguing, but exploratory in a small dataset.

⭐ MY CLINICAL PEARL

Not every form of intensification is equal.

What matters is proven survival or metastasis prevention, weighed against toxicity, patient selection, quality of life, and long-term treatment burden.

Precision + durable benefit — not just higher response rates.

🚨⭐ #ESMO26 | GU ONCOLOGY — DAY 1 HIGHLIGHTS
@urologysummit

A remarkable opening day in GU oncology — from perioperative immunotherapy to ctDNA, targeted therapy, and next-generation treatments.

The strongest message for me?

More treatment is not automatically better treatment.

🟠 1️⃣ KEYNOTE-866 | 4735O

Perioperative pembrolizumab + gemcitabine/cisplatin did not significantly improve EFS in cisplatin-eligible MIBC.

📊 EFS HR 0.88 (95% CI 0.71–1.10), p=0.1366
pCR: 44.4% vs 35.8%

💡 A higher pathological response did not translate into a significant EFS benefit.

🟠 2️⃣ ENERGIZE | 4736O

Perioperative nivolumab + chemotherapy also failed to meet its co-primary endpoints.

📊 EFS HR 0.94, p=0.6697
pCR: 39.1% vs 29.6%

Another reminder that response endpoints must be interpreted alongside durable outcomes.

🟠 3️⃣ KEYNOTE-905 / KEYNOTE-B15 | 4733O / 4734O

In exploratory KEYNOTE-905 analyses, EV + pembrolizumab favored surgery alone:

✅ OS HR 0.48
✅ pCR 58.8% vs 25.3% with pembrolizumab alone and 10.3% with surgery alone

In KEYNOTE-B15:

🧬 ctDNA clearance: 82.1% vs 66.7% with EV+pembro vs GC

ctDNA positivity correlated with poorer EFS.

Promising for risk stratification — but ctDNA-guided treatment still needs prospective validation.

🟠 4️⃣ TROPHY-U-01 Cohort 7 | 4742O

SG + EV + zimberelimab in untreated metastatic urothelial cancer:

✅ ORR 81%
✅ Median PFS 19.3 mo
✅ Median DoR 16.7 mo

⚠️ Very promising, but phase Ib, only 21 patients, and substantial toxicity.

🔵 5️⃣ CAPItello-281 | LBA56

In PTEN-deficient de novo mHSPC:

Capivasertib + abiraterone vs placebo + abiraterone

✅ Median OS 57.4 vs 46.5 mo
HR 0.82 (95% CI 0.68–0.99)

But the prespecified significance threshold was not met.

An encouraging survival signal — not yet a formally established OS benefit.

🔵 6️⃣ THE NEXT FRONTIER IN PROSTATE CANCER

☢️ PSMA-targeted radioligand therapy
🧬 AR degraders & RIPTACs
💊 Next-generation ADCs
🔬 T-cell engagers & precision immunotherapy

The challenge now is converting biological activity into durable benefit, manageable toxicity, and better patient selection.

⭐ MY POINT OF VIEW

Three principles stood out today:

✅ Survival and metastasis-prevention endpoints matter
✅ Biomarkers must move from prognostic value to therapeutic guidance
✅ Novel combinations must justify both efficacy and toxicity

The future of GU oncology is not simply intensification. It is smarter, biologically informed treatment selection.

🎯 Right strategy. Right patient. Right time.

🚨⭐ #ESMO26 | GU ONCOLOGY — DAY 1 HIGHLIGHTS
@OncoAlert

🟠🔵 From perioperative immunotherapy to molecular precision and next-generation treatments.

A remarkable opening day in GU oncology, with important lessons for both #BladderCancer and #ProstateCancer.

Here are our key 🟠 1️⃣ KEYNOTE-866 Abstract 4735O

❌ Perioperative pembrolizumab + gemcitabine/cisplatin failed to significantly improve EFS in cisplatin-eligible MIBC.

📊 EFS HR 0.88 (95% CI 0.71–1.10)
p=0.1366

pCR: 44.4% vs 35.8%

💡 Higher pathological response did not translate into a statistically significant EFS benefit.

🟠 2️⃣ ENERGIZE Abstract 4736O

❌ Perioperative nivolumab + chemotherapy also failed to meet its co-primary endpoints.

📊 EFS HR 0.94; p=0.6697
pCR: 39.1% vs 29.6%

🔬 These findings reinforce the need to interpret response endpoints alongside durable clinical outcomes.

🟠 3️⃣ KEYNOTE-905 & KEYNOTE-B15 | 4733O / 4734O

✅ The exploratory three-arm KEYNOTE-905 analysis favored EV + pembrolizumab over surgery alone.

📊 OS HR 0.48
pCR: 58.8% vs 25.3% with pembrolizumab alone and 10.3% with surgery alone.

🧬 KEYNOTE-B15:
ctDNA clearance with EV + pembrolizumab vs gemcitabine/cisplatin:

✅ 82.1% vs 66.7%

ctDNA positivity was associated with poorer EFS.

📌 Molecular monitoring offers exciting opportunities for risk stratification, but ctDNA-guided treatment decisions still require prospective validation.

🟠 4️⃣ TROPHY-U-01 COHORT 7 4742O

💊 SG + EV + zimberelimab in untreated metastatic urothelial cancer.

✅ ORR: 81%
✅ Median PFS: 19.3 months
✅ Median DoR: 16.7 months

⚠️ Phase Ib, only 21 patients, with substantial toxicity.

Promising—but not yet practice-changing.

🔵 5️⃣ CAPItello-281 LBA56

Updated overall survival in PTEN-deficient de novo mHSPC.

📊 Capivasertib + abiraterone vs placebo + abiraterone:

✅ Median OS: 57.4 vs 46.5 months
HR 0.82 (95% CI 0.68–0.99)

⚠️ The prespecified statistical significance threshold was not met.

An encouraging survival signal, but not a formally established OS benefit.

🔵 6️⃣ THE NEXT FRONTIER IN PROSTATE CANCER

Today's educational sessions highlighted major advances in:

☢️ PSMA-targeted radioligand therapy
🧬 AR degraders and RIPTACs
💊 Next-generation ADCs
🔬 T-cell engagers and precision immunotherapy

The key challenge is moving from promising biological activity toward durable clinical benefit, acceptable toxicity and better patient selection.

⭐ POINT OF VIEW

More treatment does not always mean better outcomes. And an impressive response rate does not automatically establish a new standard of care.

Today's data reinforce three essential principles:

✅ Robust survival endpoints matter.

✅ Biomarkers must move from prognostic value to validated therapeutic guidance.

✅ Novel combinations require careful assessment of efficacy, toxicity and clinical applicability.

💡 The future of GU oncology is not simply treatment intensification—it is smarter, biologically informed treatment selection.

🎯 The right strategy. The right patient. The right time.

🚨⭐ #ESMO26 | IUCS GU ONCOLOGY — DAY 3 HIGHLIGHTS

🟢 Kidney cancer took center stage today, with pivotal phase III readouts, mature adjuvant follow-up and a promising new immune target.

📊 1️⃣ LITESPARK-012 | 3586O

❌ In 1L advanced ccRCC, neither adding belzutifan nor CTLA-4 to pembrolizumab + lenvatinib improved PFS or OS.

* Belzutifan triplet: median PFS 20.8 vs 23.7 months (HR 1.08).
* More treatment discontinuations due to adverse events.

Clinical lesson: a triplet is not necessarily better than an effective doublet.

📊 2️⃣ LITESPARK-011 | 3587O

✅ After anti–PD-(L)1 therapy, belzutifan + lenvatinib maintained a PFS benefit vs cabozantinib:

* Median PFS 14.8 vs 10.8 months | HR 0.74
* ORR 53.1% vs 40.2%

⚠️ Final OS: 33.7 vs 28.6 months | HR 0.85; did not meet the prespecified significance boundary.

The combination offers an additional option, but sequencing and tolerability are key.

📊 3️⃣ KEYNOTE-564 | 3589RO

✅ Final planned analysis confirms durable benefit from adjuvant pembrolizumab in high-risk ccRCC after nephrectomy:

* 7-year DFS 57.2% vs 44.6% | HR 0.68
* 7-year OS 82.3% vs 76.6% | HR 0.70

An established standard backed by long-term disease-free AND overall survival data.

🧬 4️⃣ XmAb819 | 3588O

A first-in-class ENPP3 × CD3 T-cell engager showed confirmed response rates of ~30–46% across phase I expansion dose groups in heavily pretreated ccRCC.

⚠️ Early evidence; grade ≥3 treatment-related adverse events occurred in 72% of expansion patients. Randomized validation is essential.

🔬 5️⃣ RAMPART | 3590RO

Patient-reported outcomes highlighted early quality-of-life trade-offs with adjuvant durvalumab versus active monitoring—an important reminder that efficacy and lived experience both matter.

⭐ IUCS CLINICAL TAKEAWAY

Today delivered a powerful contrast: negative frontline triplets, meaningful post-immunotherapy PFS benefit, and durable adjuvant survival benefit.

💡 Precision is not about adding more agents. It is about choosing the right strategy, at the right stage, with the right balance of benefit and toxicity.

🚨⭐ #ESMO26 |Abstract 4735O KEYNOTE-866 🟠
Perioperative pembrolizumab or placebo + neoadjuvant cisplatin and gemcitabine in muscle-invasive bladder cancer: Phase III KEYNOTE-866
🎤 Presented by Jens Bedke 🇩🇪
@OncoAlert

🔬 Can adding perioperative pembrolizumab to vant chemotherapy improve outcomes in cisplatin-eligible MIBC?

📊 STUDY DESIGN
Phase III, randomized, double-blind, placebo-controlled trial.
👥 N=907 patients with cisplatin-eligible MIBC.

🔹 Pembrolizumab + gemcitabine/cisplatin → radical cystectomy → adjuvant pembrolizumab (n=453)
🔹 Placebo + gemcitabine/cisplatin → radical cystectomy → adjuvant placebo (n=454)

Median follow-up: 60.9 months.
📊 KEY RESULTS

❌ Event-free survival (PRIMARY ENDPOINT)
• Median EFS: 74.1 months vs NR
• HR 0.88 (95% CI 0.71–1.10)
• p=0.1366
➡️ No statistically significant EFS improvement.

📈 Pathological complete response (pCR)
✅ 44.4% vs 35.8%
• Estimated difference: +8.9 percentage points (95% CI 2.5–15.1).
⚠️ Higher pCR did NOT translate into a statistically significant improvement in the primary EFS endpoint.

📊 Overall survival
• Median OS: NR in both arms
• HR 1.00 (95% CI 0.79–1.27)
➡️ No observed OS advantage.

pCR and OS were not formally tested for statistical significance under the hierarchical testing strategy.

🏥 Surgery
• Underwent cystectomy: 91.6% vs 87.9%.
⚠️ SAFETY
• Grade 3–5 treatment-emergent AEs: 80.2% vs 75.1%.
• Grade 3–5 immune-mediated AEs/infusion reactions: 8.9% vs 1.8%.
🔬 CONCLUSION
KEYNOTE-866 did not meet its primary EFS endpoint. Despite increased pCR rates, perioperative pembrolizumab added to neoadjuvant cisplatin/gemcitabine did not demonstrate a significant EFS benefit.
⭐ MY POINT OF VIEW | CLINICAL PEARL
A higher pathological response rate is encouraging, but it is not enough to redefine a standard of care.

In curative-intent MIBC, durable disease control and survival must drive treatment decisions.
📌 These findings do not support routinely adding perioperative pembrolizumab to neoadjuvant gemcitabine/cisplatin in cisplatin-eligible MIBC.
💡 More treatment does not necessarily mean better outcomes. The real challenge is identifying which patients benefit from intensification—and which do not.
