Secretory carcinoma of the breast is a rare (<0.2%) low-grade triple-negative carcinoma defined by the ETV6::NTRK3 fusion.

🔬 Key clues: bubbly eosinophilic secretions, S100+, ER/PR/HER2−, and PAN-TRK positivity.

Remarkably, it occurs from childhood to older age. #breastpath

Classic histology of epithelioid hemangioendothelioma(EHE) in the lung.

One of those diagnoses that is easy when you know the morphology, but can be a real diagnostic challenge when you don’t.

IASLC expert panel proposes an updated pulmonary carcinoid/NET classification ( Expected to be included into the forthcoming WHO thoracic blue book)

1. Keep “carcinoid” as the main diagnostic term for thoracic clinicians, while moving toward the “NET” nomenclature used in other a diagnostic criterion. 5% threshold is pulmonary specific
3. Formally recognize rare carcinoid/NET G3 tumors (higher proliferation, but molecularly closer to NETs than to high-grade NECs)

Secretory carcinoma

Almost all secretory carcinomas harbor ETV6-NTRK3
translocations and FISH for ETV6 is a useful ancillary study to confirm the diagnosis

Secretory carcinoma is a subset of triple-negative breast cancers with a favorable prognosis. udies have reported instances of recurrence and metastasis

Updates from the WHO Classification of Breast Tumors, 6th Edition (not published yet)

The WHO 5th edition recommended that encapsulated papillary carcinoma (EPC) with high nuclear grade, increased mitotic activity, and/or a triple-negative or HER2-positive phenotype be graded, invasive disease

In the 6th edition, the WHO now recognizes the existence of rare forms of EPC with high-grade features or ER negativity that may still be categorized as EPC
Such tumors may be encountered on core biopsy; classifying them as invasive could lead to inadvertent neoadjuvant therapy

The vast majority of EPCs should be low- to intermediate-nuclear grade and ER-positive
