Very important data from PARTNER-3 presented at #TCT2025 👇

At 7 years, the composite endpoint (death, MI, stroke, or rehospitalization) occurred in 34.6% with TAVR vs 37.2% with surgery (difference -2.6%; 95% CI, -9.0 to 3.7). No differences between the two groups in stroke, valve deterioration, endocarditis, and revascularization.

Perhaps the most informative figure ⬇️: the vital status sweep analysis.
After accounting for patients initially lost to follow-up but later confirmed dead, the apparent late excess mortality with TAVR disappeared, highlighting the critical value of complete follow-up ascertainment.

Overall, TAVR remains a very safe and effective option for low-risk patients with aortic stenosis at long-term follow-up.

Presented at #TCT2025:

In low-risk patients with severe aortic stenosis, transcatheter and surgical aortic-valve replacement had similar 7-year survival and valve durability outcomes, with no evidence of differences in composite end points. Full PARTNER 3 trial results: nej.md/4nieHfx

#proglide and #manta allow safely seal #TAVR access entry site so effectively that surgical access looks today unacceptable in the majority of pts. Yet, vascular complications continue to occur so that improvement in anatomy-based device selection is deserved. #TAVI
🔥 Just published in @ehjopen @escardio

In 9259 patients undergoing TAVR:
🔸MANTA ⬇️ bleedings and vascular complications
🔸ProGlide ⬇️ vascular complications in RCT/PM trials
🔸ProStar performed poorly compared to MANTA and ProGlide

In the IMP-IT registry, device-related complications rate after #Impella implantation was 25.6%, with cardiogenic shock being the strongest predictor. Device-related complication wasn't an independent predictor of 1y mortality.

2/3 Clear reductions in unstable angina and angina-free status w/revasc. No diff in HF or stroke. All-cause death nearly identical between groups, with tight CI (RR=0.99, 95% CI 0.90-1.09). Net results favor revascularization in stable ischemic heart disease.

I don't know where this "in between" lies, but, anyway:

"The old paradigm of ‘SAVR if possible, TAVI if necessary’ no longer holds. Equally valid might be a new paradigm; ‘TAVI if possible, SAVR if necessary’. The truth is probably somewhere in between."

Final 5-yr ABSORB analysis. Increased risk of BVS vs Xience EES ends at 3 years (time of complete bioresorption)- thereafter event rates tended to be lower with BVS. If new gen scaffolds have comparable results to DES w/i 3 yrs, BVS might be preferred to DES for many CAD pts.

#TAVR for low risk patients approved by FDA today!

Recent NCD lowered requirement from 2 cardiac surgeon consults to just 1.

Should it be a requirement that a patient with aortic stenosis also see an interventional cardiologist to discuss TAVR option before having SAVR?

¡¡¡Se acabó!!!! 16/8/2019 FDA Approves TAVR in Low-Risk Patients.
Risk stratification for TAVR treatment is becoming obsolete and heart teams will likely need to assess treatment options based on anatomical characteristics, concomitant risk factors, and also patient preference
