Congratulations 🎉 @crfheart @TCTConference
Nearly 13K of you showed up, and #TCT2026 delivered. The shift was unmistakable: from devices to equity, best practices at center stage and AI in the spotlight. To our next gen of leaders: be curious, not judgmental. Proud of the best team in the industry! @crfheart @triciarawh

Left BASILICA today in 73 YO
Anatomy was borderline. Left coronary height was 10 mm and left sinus was 27 mm. For this first valve, you could probably get away without modification. But 73 YO will likely outlive this valve. So the question isn't whether coronary is at risk now. l have options when we need TAV-in-TAV. I think we should be more liberal with leaflet modification in younger patients with borderline anatomy.
One thing worth pointing out. After the first traversal, TEE showed wire went through aortomitral curtain into the LA. You have to catch this before advancing microcatheter or go after it with the snare. That's also why we keep the snare in the LVOT and not down in the LV. If the wire has gone through the LA and across the mitral valve, it won't come through a snare sitting in the LVOT. If we push the wire and snare deep into the LV to grab it, we can end up snaring a wire and lacerating aortomitral curtain.
So pulled back, used back end of J wite to stand AL3 guide less basal and traversed again, snared it with a 20 gooseneck, formed the flying V and lacerated. Post-laceration TEE shows a nice V splay of the leaflet.

Great trial and important data. It proves what many of us know, if we fix the valve before the myocardium gets hit, patients do better. But the win is mainly HF events and quality of life, mortality was not different. Still meaningful but lets be honest about what it showed.
My bigger worry is what comes next. If we start treating moderate AS earlier, these patients will outlive their valves, no question. Our valves are good but not perfect and durability is still a big issue. We are going to see a lot of TAV in TAV in TAV, and I am not sure we as a field are ready for that.
Lifetime management has to start at the first procedure. CT planning for future redo, anticipating sinus sequestration and coronary obstruction risk, very low threshold for leaflet modification, commissural alignment, and choosing the right valve and size thinking about the second one. And for younger pts, less than 70 YO at least in the US, surgery first should still be on the table.
Exciting time for structural but we need to be careful how we apply this.

💡@TCTConference Thank you for these summary slides projected during the discussion

✅ @SanjitSJolly 100 U/kg showed no advantage over 70 U/kg in elective PCI: a lower dose with ACT-guided top-up is a reasonable default strategy.
