Sharing a practical LIVING @OpenMedicineHQ algorithm Multiple Myeloma at First Relapse: Treatment by Drug Sensitivity and Clinical Context (Last updated TODAY, November 6, 2026) by @ManniMD1
💭Key point: Finite-duration bispecific therapy, including approximately 6–12 months in selected contexts, is an individualized expert-practice or investigational stopping strategy—not a universal standard.

Sharing a practical @OpenMedicineHQ algorithm “Primary Plasma Cell Leukemia (PCL)” by @HadidiSamer (Last updated October 16, 2026).
Key points:
- Primary PCL: Confirm diagnosis and assess for presence of t(11;14)
- VD-PACE (in most patients):
* Consider one cycle of VD-PACE at time of diagnosis (if possible) for cyto-reduction and immediate disease control.
* Given for 2 cycles prior to tandem ASCT or as 1 cycle followed by 4-6 cycles of 4-drugs prior to tandem ASCT.

Sharing a practical @OpenMedicineHQ algorithm “Toxicity management for esophageal stricture” by Nina Sanford, MD @NiuSanford
(Last updated October 16, 2026).
Key points:
- Before therapeutic endoscopy, confirm benign disease with nd biopsy of a new stricture
- Use contrast esophagram or CT when the stricture is long, angulated, complex, completely obstructing, or associated with a suspected fistula
- Radiation strictures are delayed toxicities and may become clinically apparent months or years after treatment. Timing is variable, so the node is directionally accurate rather than a strict diagnostic interval.
- Dilation: usually requires multiple sessions (~3), done gradually to reduce risk of perforation —> Can stent if refractory.

#ESMO26 Proud to share results from the TERESA study with Drs. @peters_solange @JoeChangMD et al: Real world treatment patterns and outcomes in pts with stage I-II NSCLC who did not undergo resection. In this Flatiron real world analysis, 40% of pts with stage I-II NSCLC did not of those pts received SBRT. Study of systemic therapy in this pt population represents an important opportunity to improve outcomes and is feasible, based on the size of the real world population.

1/ @PTarantinoMD on pathological and clinical landscape of HER2-low breast cancer:
x.com/PTarantinoMD/s…
6 years ago, I had the chance to publish a paper that would end up changing my academic life
It was a review article exploring the surprising activity of certain ADCs in “HER2-low” breast cancer, a newly emerging entity whose relevance was still uncertain.
We tried to connect 2-low was a subtype vs just a target, wondering whether this concept could one day translate into meaningful benefits for patients.
6 years later, that article has reached 1,000 citations. My career has gifted me more than I could ever have imagined. More importantly, what was once an intriguing scientific observation has become a clinical reality: countless patients with HER2-low breast cancer now live longer thanks to T-DXd.
A reminder of how far science can take us in just a few years.
Grateful to everyone who has been part of this journey!

Sharing a practical @OpenMedicineHQ algorithm “Relapsed or Resistant Chronic-Phase Chronic Myeloid Leukemia Treatment” (Last updated October 27, 2026) by @TalhaBadarMD
Key points:
- Assess the BCR::ABL1 molecular response trajectory and is an inadequate response or loss of a previously achieved response.
- Do not base a treatment switch on a single unexpected PCR result alone. Review the molecular trajectory together with adherence, dose interruptions or reductions, drug interactions, comorbidities, and potential laboratory variability.
- Clinically important response values include >10% at 3 months when confirmed, >10% at 6 months, >1% at 12 months, or loss of a prior response
