Can patients be better prepared before cardiac surgery?
Prehabilitation seeks to improve resilience before surgery through three complementary components: exercise, nutritional support and psychological preparation. Although the concept is established in several non-cardiac role in cardiac surgery remains less certain, particularly given the burden of frailty and reduced cardiorespiratory fitness among many patients.
In this BMJ Heart podcast, Professor James Rudd (@jhfrudd) is joined by Professor Enoch Akowuah (@EnochAkowuah1 on X), consultant cardiac surgeon and Newcastle University academic. They discuss the evidence for prehabilitation before cardiac surgery, the practical challenges of implementing it, and the plans for a major UK multicentre trial.

Around a third of patients don't respond to cardiac resynchronisation therapy. And up to 10% of implants fail to reach the target vessel at all.
Conduction system pacing aims to fix both — by recruiting the heart’s own His-Purkinje network instead of working around it.
