🦠 VIRAGE | Nature Medicine
VCN-01 (zabilugene almadenorepvec) is an oncolytic adenovirus engineered to express soluble hyaluronidase, degrading the hyaluronic-acid-dense stroma that makes pancreatic tumours impenetrable to chemotherapy.
In this randomized phase 2b: 2 IV doses GnP alone in 1L metastatic PDAC.
🔀 ITT (53 vs 48): mOS 10.6 vs 8.6 mo, HR 0.69, P=0.196 ➖ not significant
📈 Full analysis set (48 vs 48): mOS 10.8 vs 8.6 mo, HR 0.57, P=0.055; PFS 7.0 vs 4.6, HR 0.55, P=0.011
⏱️ Duration of response 11.2 vs 5.4 mo, HR 0.22, P=0.004
🔴 18-month survival 31.1% vs 8.5%
➖ ORR, DCR and CA19-9 all flat
⚠️ Serious virus-related events 22.6%
The interesting thing is not that tumors shrank more (they did not). It is that responses lasted more than twice as long (DOR 11.2 vs 5.4 months, HR 0.22), which is exactly what you would predict if opening the stroma keeps chemotherapy reaching the tumor.
The caveat is equally clear: ITT overall survival was negative at P=0.196, and the win sits in a full analysis set that is five patients smaller in the experimental arm.
Needs phase 3 confirmation. REMEMBER: Randomized Phase 2 trials in in closely monitored academic centers may be subject to selection effects that become less pronounced in larger, more diverse populations.

1️⃣ Metastatic pancreatic cancer: in the phase 2b VIRAGE trial, adding VCN-01, an IV oncolytic adenovirus (a modified virus designed to target tumors and reshape their surrounding environment), to gem/nab-paclitaxel led to mo, HR 0.55) and overall survival (10.8 vs 8.6 mo, HR 0.57). Needs phase 3 confirmation.
2️⃣ MSI/dMMR solid tumors: a first-in-class covalent WRN inhibitor (RO7589831) was tested in heavily pretreated patients, 95% after immunotherapy. Disease control was 74% and confirmed responses 11% among evaluable patients, with most benefit being stable disease.
Two very different strategies: reshaping the tumor environment and exploiting a cancer-specific genetic vulnerability.
