The first @APCCC_Lugano 2015 did not mention PSMA PET once!

CT plus bone scan were deemed sufficient to define metastatic disease (77% consensus for M0).

+11 years: the #APCCC26 report on high-risk localised and biochemically recurrent disease is out in @EUplatinum, with 67 SMA PET or next-generation imaging. I pulled those votes into a single wheel to highlight there are many areas of disagreement amongst the experts and much work to do!

LuCAB at #ESMO26 - Combination Lu-177 PSMA-617 + cabazitaxel in mCRPC, accepted in @TheLancetOncol

Adding taxane chemotherapy to LuPSMA may overcome heterogeneous PSMA expression and act as a radiosensitiser.

35 pts, post-docetaxel + ARPI:
🧪 No DLTs: cabazitaxel 20 mg/m2 + AEs 24%, no G4/5
📈 RP2D (n=26): PSA50 73%, rPFS 10.9 mo, OS 16.5 mo

Impressive outcomes in an unfavourable cohort (29% M1c).

Randomised trials needed: chemo + LuPSMA together or in sequence?

🙏 Led by @LouiseKostos & @AzadOncology with @ButeauJames , Grace Kong, @pj_physics, @DrBenTran, @MHaskali, @declangmurphy @PeterMacCC and @drlouiseemmett @StVincentsSyd

Above all, thank you to the patients and families who took part.

Funded by @PCFnews through @pros_tic , with PSMA-617 supplied by @Novartis plus additional support.

✍️ the story continues in the new era of RLTs. 225Ac-DOTATATE is an effective therapy in GEPs tumors after progressing to SOC and Lut177 based-therapy! ..what about 225-Ac-PSMA in prostate cancer ??👇

Proud moment at #EANM26 @EANM_NucMed in Vienna watching Dr Cameron Pain, our Total Body PET Lead Medical Physicist, present the fast-acquisition results from our EQUIP study (NCT07263815) on the @GEHealthCare Omni 128 total-body PET/CT at Peter MacCallum Cancer Centre - the ion of this system.

Can a 1 minute Total Body PET (LAFOV: large axial field-of-view) maintain or improve diagnostic quality compared with conventional short axial FOV (SAFOV) PET/CT?

25 patients were imaged on both systems. Overall diagnostic quality rated “excellent” in 0% of SAFOV scans, 57% of LAFOV 1-min scans and 100% of LAFOV 5-min scans.

Mean diagnostic quality 3.9 (SAFOV) vs 4.5 (1-min) vs 5.0 (5-min); sharpness and lesion conspicuity 3.7 vs 4.3 vs 5.0 100% of 1-min LAFOV scans rated excellent, good or acceptable.

Congratulations to the team Dr Alicia Corlett, @pj_physics , central readers A/Prof Grace Kong, Dr Ramin Alipour, Dr Aaron Wong, & Cancer Imaging Director Prof Stephen Stuckey.

Pushing Terbium-161 PSMA Theranostics to 9.5 GBq - VIOLET Clinical Trial results at the #EANM26 @EANM_NucMed

Terbium-161 emits beta radiation like lutetium-177, plus ultra-short range Auger and conversion electrons that may better target micrometastases. Our first-in-human d safety up to 7.4 GBq (Lancet Oncology 2025).

So we asked: can we go higher? 12 men with mCRPC, 9.5 GBq, up to 6 cycles:
- Remarkably well tolerated: just 1 grade 3 toxicity (anaemia)
- PSA50 response 75%, PSA90 response 33%
- Median PSA-PFS and rPFS not reached

The answer is yes - and we’re not stopping ... Cohort C, a dose-dense cohort with shorter intervals between early cycles, is now underway.

Huge thanks to @PCF_Science Young Investigators @ButeauJames , @LouiseKostos and @AzadOncology , and to the whole @PeterMacCC @pros_tic team who made this investigator-initiated trial possible.

Grateful to @Isotopia_Global and the for their support

Huge turnout for @thomashopemd "Rethinking FAPI targeted therapy" @EANM_NucMed #EANM26
Best overview talk in the FAPI landscape I have heard. The lowish uptake and low retention compared to PSMA or DOTATATE is a big challenge but one that many groups are trying to address

Nearly every presentation delivered new data, evolving perspectives, and game-changing insights into prostate cancer care.

What a difference two years makes in theranostics! Heading into #ProsTIC26, I’ll admit I found myself asking some tough questions: Are there already too s? Has the field really shifted that much? Do we actually need another ProsTIC?

Standing in Melbourne last week alongside over 500 participants from around 25 countries, the answer was a resounding yes. The pace of progress since #ProsTIC24 is mind-blowing.

The week proved that bringing so many different disciplines into one room is essential for pushing patient outcomes forward.

From quite intense clinical debates, learning from patient experience, discussing new scientific approaches like Terbium-161 and Lead-212, to an unforgettable 80s-themed celebration, the momentum in this space seems unstoppable.

Check out the video below for a recap of the highlights - and get ready, because with the way this field is expanding, #ProsTIC28 can't come soon enough!

New review with Wolfgang Fendler, David Kersting, @CalaisJeremie & @thomashopemd on the phase 3 theranostic pipeline: 30 RPT trials launched in the last 5 years, including by PSMAcTION, STAMPEDE2, PSMA-DC, PSMAddition, NETTER-3, ACTION-1 and COMPOSE.

The next frontier is earland new targets and but trial designs still need to embrace dosimetry and adaptive, image-guided treatment.
