Late-breaker at #TCT2026: P4 randomized 1,105 pts to CT-guided vs IVUS-guided PCI.
12-mo target-vessel failure 4.1% vs 4.6% – noninferior.
A CT roadmap made before the cath lab matched the gold standard of invasive imaging.
Simultaneously in @NEJM 🧵

Fine print: noninferiority margin 4.5 pts vs observed TVF of 4.6% – generous. Periprocedural MI drove >half of events. Contrast/radiation counted for the invasive procedure only, not CCTA. Stable pts, FFR-CT ≤0.80, no left main.
Still – CT is becoming the PCI roadmap.
