At @HopkinsMedicine Johns Hopkins Bayview Medical Center in Baltimore, Maryland, an employee uploaded a spreadsheet containing protected health information (PHI) into an unauthorized AI tool, exposing information associated with 3,182 patients.

AI adoption is fundamentally a transformation of how works gets done. Employees need to understand what the technology will do, how their responsibilities will change, where human judgment remains essential, and how an AI project's success benefits them.

Production monitoring is not optional. Once deployed, AI agents require:

• observability 👀
• performance evaluations
• incident response plan
• cost monitoring/token consumption
• ongoing governance

A successful launch is just the beginning of the operational lifecycle.

Integration is where much of the real work begins in an AI project.

A successful demo may operate in isolation. Production agents must interact reliably with existing CRMs, ERPs, scheduling platforms, databases, APIs, identity systems, and existing business workflows.

Autonomy must be proportional to risk.

Not every task should be fully autonomous.

Data quality matters as much as data access.

Giving an AI agent permission to retrieve inconsistent, outdated, or incomplete data doesn't make it useful.

Data readiness should be evaluated before implementation.

Good judgment isn't the ability to identify every potential problem before entering a relationship.

It's the ability to recognize patterns, communicate boundaries, observe how someone responds, and make adjustments, when a relationship consistently fails to meet your needs.
