The I-C-E clamping order during Carotid Artery Endarterectomy!
1I – Internal Carotid Artery (ICA): Clamped first. This isolates the cerebral circulation, preventing loose emboli or plaque fragments from traveling to the brain while the other vessels are handled. This is small, gentle bulldog clamp or temporary aneurysm clip.
2C – Common Carotid Artery (CCA): Clamped second. This stops the main high-pressure blood flow coming up from the heart. Surgeons often use a larger, angled vascular clamp (such as a DeBakey clamp) for this wider vessel.
3E – External Carotid Artery (ECA): Clamped third. This stops backward blood flow (back-bleeding) entering the surgical field from the face and scalp vessels.
The Reversal Sequence (E-C-I)
Once the plaque removal (endarterectomy) and arterial patching are complete, the clamps are removed in the reverse order (E-C-I) to safely flush out any air bubbles or debris before restoring direct blood flow to the brain:
•External Carotid clamp is released first.
•Common Carotid clamp is released second. Blood from the heart is purposefully directed into the external carotid artery first to flush out any debris into the facial circulation.
•Internal Carotid clamp is released last, safely restoring clean, uncompromised blood flow back to the brain.
